DISTRIBUTOR AGREEMENT:
I hereby apply to become a Painex Distributor in accordance with the following terms and conditions. I shall become a distributor upon the acceptance of this application by Painex, Inc. As a Painex distributor, I shall have the right to sell the products produced by Painex, Inc. in accordance with Painex, Inc. Policies and Procedures.

I understand the nature of the Painex program, and my desire to become a distributor is made with full knowledge of the following facts: (a) I am at least 18 years of age, and (b) I may cancel this contract for any reason and at any time upon written notice to Painex, Inc.

I have read and understand this application and agree to abide by all the rules and regulations and terms and conditions set forth herein, and in any updates thereto.
